Attendance Permits

Requests to Transfer Student Records:  Records Request

If you are enrolling, please send this request to your prior school to have all documents sent to MBMS, Rglasgow@mbusd.org

 

 

Withdrawals: Withdrawal Form
If you wish to withdraw from Manhattan Beach Middle School, please fill out the Withdrawal form and returned to:
Rglasgow@mbusd.org.  Withdrawals can only be processed for students/parents who have completed and signed the withdrawal form indicating which school the student is going to next.

 

 

Transcript Requests- Private School Applications: 

If you apply to private schools and need transcripts or other documents forwarded, please provide the listed email to the schools (they will send the request directly to us) or send the transcript request to: Mmarquez@mbusd.orgAny recommendation requests need to be emailed directly to the teacher or administrator

*Administration and Teachers need a minimum of two weeks’ notice in order for us to send the records by the due date.

 

Entertainment Work Permits:  Please complete the top portion of the Entertainment Work Permit Application and email to: Mmarquez@mbusd.org.  You’ll need to allow two days to research our records and complete the form.  Completed applications will be available for pick up, two days after the permit is received.
